HOW TO ENROL

Either:

• Post the completed Enrolment Form, with your cheque made out to Leeds City Council, to:
  The Community Education Programme, Prince Henry's Grammar School, Farnley Lane, Otley, LS21  2BB
  OR:
• Bring the completed Form and the appropriate fee to one of our Enrolment Sessions.

Means-tested benefits concession applies to (at time of publication):

• Income-based job seekers allowance
• Income support
• Pension credit
• Income-based employment support allowance

Proof of eligibility must be shown if enrolling in person. For postal enrolments please bring to first 2 weeks of course.

VIABILITY OF COURSES, REFUNDS and DATA PROTECTION

• All courses will run provided that they attract viable numbers.
• Full refunds are only given if a class does not run. Part refunds are paid if a class closes within the first three weeks of the course due to low numbers.
• We store your name, address and telephone number in order to contact you regarding the courses. Your telephone number is used to contact you in case of tutor illness or to pass on urgent messages.
  Your name and address will only be used by Prince Henry’s Community Education Programme in the context of the courses.
